'Almost identical' accident happened at Dreamworld two years earlier, operator fired

An inquest into the death of four people on a Dreamworld water ride hears an employee operator’s employment was terminated in 2014 because of a safety issue on the same ride and a new operator was never told by management what exactly had occurred.
